Start Young directors, please use this page to access the information and forms needed to support your staff in completing assessments as well as to support your families in finding tuition support through the child care subsidy advocates. Assessments
Early Learning Program Profile (ELPP) Information
Consent Forms
Have each family complete at enrollment, hold on site for your records. Stress with families that individual information or data will never be shared – names are removed and data is only shared collectively.
Intake Forms
Have each classroom complete annually, and each family complete (one for parent, one for child). Turn into United Way (Jennifer/Andrew) before assessments are done. All children and parents need new intake forms, even if they have been completed previously.
